Attorney Invoicing

The Board of Parole Hearings (BPH) invoice forms to bill for services are available below. The BPH 1076 invoice is used for suitability hearings and the BPH 1076-OMHD invoice is used for Offenders with Mental Health Disorder proceedings.

• BPH 1076: Attorney Invoice (rev 08-23-2023)
• BPH 1076: OMHD Attorney Invoice (rev 1-2022) – for hearings on or after January 1, 2020

• CDCR-EFT001 Electronic Funds Transfer (EFT) Payment Enrollment Form

IF YOU ARE WORKING WITH BPH FOR THE FIRST TIME AS AN ATTORNEY, you must also submit a Payee Data Record Form and a Supplement Vendor Payee Data Record Form so you can be paid. Attorneys already on record with BPH need not submit these forms.
